Research on cannabis for social anxiety is small in scale. Most human studies test CBD, not THC. Results change with dose. No health agency has approved cannabis or CBD to treat anxiety.

Human studies on CBD

A 2011 double-blind trial in Brazil gave 24 adults with social anxiety disorder a single 600 mg dose of CBD or a placebo. CBD lowered anxiety scores before a public speaking test. The trial was small and tested one dose.

A 2019 trial with 57 men found that 300 mg of CBD reduced anxiety in a simulated public speaking test. Doses of 150 mg and 600 mg did not show the same result. The authors described an inverted U-shaped dose curve.

A 2019 case series in Colorado followed 11 patients who took about 21 mg of CBD per day in divided doses. Ten reported less anxiety at the first monthly visit. The study had no placebo group.

THC and anxiety

THC binds CB1 receptors in the brain. NIDA reports that high doses can cause panic, paranoia, and a rapid heartbeat. A 2019 study in Lancet Psychiatry gave THC to healthy volunteers and recorded more psychotic symptoms and anxiety than with placebo. CBD did not block those effects at the doses tested.

Doses used in studies

Single-dose studies used 300 mg and 600 mg of CBD. The case series used 21 mg per day, split into smaller servings. Store products often hold 10 mg to 50 mg per serving. No clinical guideline sets a CBD dose for social anxiety.

What to check on a label

  • A certificate of analysis from a third-party lab for that batch.
  • Total THC at or below 0.3% for hemp products.
  • Tests for pesticides, heavy metals, leftover solvents, and microbes.
  • A batch number that matches the certificate.

A 2017 study in JAMA tested 84 CBD products sold online. About 26% contained less CBD than the label claimed. About 43% contained more. THC appeared in 18 products.

Side effects and interactions

Reported CBD side effects include drowsiness, diarrhea, appetite change, and raised liver enzymes. The FDA warns about liver injury at high doses. CBD inhibits the CYP3A4 and CYP2D6 enzymes, so it can raise blood levels of other drugs. Named interactions include clobazam, valproate, warfarin, clozapine, and some HIV drugs.

Legal status

Hemp-derived CBD with 0.3% THC or less is legal at the federal level under the 2018 Farm Bill. State rules differ. Cannabis with more THC stays illegal under federal law, though many states allow medical or adult use.

When to talk to a clinician

Social anxiety disorder affects about 7% of US adults in a 12-month period, per NIMH. First-line care is cognitive behavioral therapy and SSRI drugs. Talk with a clinician before you swap out any prescription.
