Clinical reviews do not place cannabis or CBD in the first line of back pain care. Guidelines from the American College of Physicians start with heat, exercise, physical therapy, and NSAIDs. The FDA has approved one cannabis-derived drug, Epidiolex, and its use is epilepsy, not pain. Systematic reviews report small effects for cannabinoids in chronic pain. Evidence that targets back pain is thin.
Back pain by the numbers
About 80% of US adults have low back pain at some point. Most acute cases improve in 4 to 6 weeks without prescription drugs. A minority go on to have pain that lasts 12 weeks or longer, which is the cutoff for chronic low back pain.

CBD and THC are not the same
- CBD, or cannabidiol, does not cause intoxication at typical retail doses. It comes as oil, capsule, gummy, and topical.
- THC, or delta-9-tetrahydrocannabinol, binds CB1 receptors in the brain and causes impairment.
- Hemp CBD may contain no more than 0.3% THC by dry weight under the 2018 Farm Bill.
- Topicals act on skin and joints. Little CBD from a cream reaches the blood.
- Smoked and vaped products reach the blood in minutes. Oral products pass through the liver first and lose potency.
What trials show
A 2018 Cochrane review of cannabis-based medicines for chronic neuropathic pain found moderate-quality evidence of a small pain reduction. About 1 in 10 people reported at least 30% relief beyond placebo. A 2017 National Academies report found substantial evidence for cannabis in chronic pain, and most of those studies looked at nerve pain. NIH states that CBD research is at an early stage. Trials on CBD by itself for back pain are few and small. Most studies use THC or a THC-CBD mix.
Dose and product checks
- No standard dose exists for back pain. Epidiolex doses of 5 to 20 mg per kg per day apply to epilepsy, not pain.
- Oral CBD absorption runs near 6% in some studies. Food can raise it.
- Labels do not always match content. Ask for a third-party lab report, called a COA, with cannabinoid levels and tests for pesticides, heavy metals, and solvents.
- Track dose, time, and pain score in a log for 2 to 4 weeks.
Risks and drug interactions
- CBD blocks the CYP3A4 and CYP2C19 enzymes. It can raise blood levels of clobazam, warfarin, and other drugs.
- About 9% of adults who use cannabis develop dependence. Risk is higher for people who start as teens.
- THC impairs driving for hours. It can show on a drug test weeks after use.
- Heavy long-term use can cause cannabinoid hyperemesis syndrome: repeated vomiting and abdominal pain.
- Vitamin E acetate in vape cartridges caused EVALI lung injuries, per CDC.
- Federal guidance advises against cannabis and CBD in pregnancy and breastfeeding.
US law
Hemp CBD with 0.3% THC or less is legal under federal law. State rules differ. As of 2024, 38 states and the District of Columbia run medical cannabis programs. The FDA has not approved cannabis or CBD for pain. Selling CBD as a pain treatment with a disease claim breaks federal law.
