Short answer

Alcohol and cannabis both slow reaction time, blur judgment, and cut coordination. Used together, they raise impairment, heart rate, and the chance of nausea or panic more than either one alone. CBD does not cause intoxication, but it competes for some of the same liver enzymes that break down alcohol and prescription drugs. No US agency has approved a cannabis and alcohol combination for any use.

What each substance does

Alcohol is ethanol. It acts as a central nervous system depressant. At higher doses it slows breathing, pulse response, and movement.

related article

Cannabis holds more than 100 cannabinoids. THC is the compound that causes the high. CBD does not cause intoxication.

Shared liver pathway

The liver clears alcohol through alcohol dehydrogenase, aldehyde dehydrogenase, and CYP2E1. CBD inhibits CYP2C19, CYP2C9, CYP2D6, and CYP3A4. THC is processed by CYP2C9 and CYP3A4. When two substances use the same enzymes, one can build up in the blood. This is the same mechanism behind grapefruit and statin warnings.

Why timing matters

Blood alcohol peaks about 30 to 90 minutes after a drink on an empty stomach. CBD plasma levels peak about 1 to 3 hours after an oral dose. Overlapping peaks raise the odds of an interaction.

Effects reported from mixing

  • Higher blood alcohol levels when cannabis is used before drinking
  • Faster heart rate and a drop in blood pressure on standing
  • Nausea, vomiting, and dizziness
  • Anxiety, panic, and confusion
  • Worse driving performance than either substance alone

Legal status in the US

Alcohol is legal for adults 21 and older in all 50 states. Cannabis remains a Schedule I drug under federal law. State laws differ. Hemp-derived CBD with less than 0.3% THC is legal under the 2018 Farm Bill. The FDA has approved one CBD drug, Epidiolex, for two seizure disorders. It has not approved CBD for other uses.

Risk notes

  • Do not drive after using either substance.
  • Use caution with sedatives, opioids, and antidepressants.
  • Ask a pharmacist about your prescriptions and CBD.
  • Keep both products away from anyone under 21.

Bottom line

Mixing alcohol and cannabis increases impairment and side effects. CBD adds a liver enzyme variable. Data on long-term combined use is limited, and study results vary by dose, route, and product.
